Refining Target Volume Coverage after Parotidectomy for Cutaneous Squamous Cell Carcinoma: Omission of the Cervical Neck from the Radiation Field

PurposeFor patients without pathological evidence of cervical disease after neck dissection for cutaneous squamous cell carcinoma involving the parotid region, inclusion ipsilateral in post-parotidectomy radiation volume is routinely performed. We report our experience with selective avoidance undergoing postoperative to bed.Methods and MaterialsFrom January 2014 December 2023, a total 30 consecutive underwent parotidectomy area. All had previously confirming N0 disease. Treatment was delivered using intensity-modulated radiotherapy median dose 60 Gy (range, 56 66 Gy). The target volumes included bed only, deliberate neck. pathologic tumor size 3.3 cm 0.2 9.4 cm). Final evaluation showed positive microscopic margins 8 (27%), perineural invasion 17 (57%), facial nerve involvement 6 (20%).ResultsThere were no isolated nodal failures. One patient developed an recurrence at approximately months completion therapy. This occurred 2 subsequent development local recurrence. 5-year actuarial rates (parotid) control, overall survival 87%, 97%, 76%, respectively.ConclusionOmission from does not compromise control pathologically region. Practical implications are discussed.
